REMEMBER TO FILL OUT THE OF-310 HOTEL COMMENT FORM

Just a reminder, to fill out an OF-310 form for any Hotel Comments whether good or bad. It is no problem contacting your LEC Officers, however, I ask you to follow up in writing as well. The only avenue for your comments to reach the appropriate US Airways personnel (Travel Services Purchasing) would be via the OF-310. Just to let you know, Perry Lopez, your AFA MEC Hotel Committee Chairperson has been under a tremendous work load and working diligently on behalf of AFA to secure safe, convenient and comfortable accommodations for each of us. His tasks are becoming more and more difficult as we strive to emerge from our economic woes. It is important after working long days that we meet these standards of accommodations so that we may be better prepared to serve our customer in our everyday duties on behalf of US Airways. 

There are 3 options to inform the Company and AFA of your hotel comments or problems; the online form at www.afausairways.org/FAForm.html, the OF310 available on US Airways "theHub", and by filling out the OF-310 available in the crew room and dropping it in the appropriate box.
