2014 Benefits Enrollment Now Open

You can check to see if your current providers participate in the network available in your state by logging on to and enter " usairways " as the EMPLOYER ID for Blue Cross Blue Shield or for United Healthcare.

PHX Based Flight Attendant Benefit FAQs

PHX Based Flight Attendants - Now through November 20th, MEC Benefits Co-Chair Eva De Castro, along with representatives of AFA US Airways Benefits will be available in the PHX Check-In Room to assist with any 2014 Benefits Enrollment questions during the following times:

Monday through Friday from 06:30 until 1300 and 16:30 until 21:00

Saturday & Sunday from 07:00 until 13:00

PHX Based Flight Attendants - Disability Plan Changes

As a result of the single agreement there will be some changes to your disability coverage beginning January 1, 2014.

  • Short-Term Disability (STD): STD is available for a two-year bridge period through February 2015. The duration for STD will be 120 days beginning January 1, 2014. This is only available to Flight Attendants who were based in PHX on February 28, 2013.
  • Long-Term Disability (LTD): You will move to the CLT/DCA/PHL-based Flight Attendant LTD plan effective January 1, 2014. Contributions to the plan are 25% of the cost of the plan, not to exceed $20/month. For 2014, the contribution will be $15/month.
PHX-based Flight Attendants who are disabled prior to January 1, 2014 will be covered by the current PHX-based Flight Attendant STD/LTD plan for the duration of their disability.

A new Life/Accidental Death and Dismemberment (AD&D) plan will go into effect on January 1, 2014 for all Flight Attendants. Below is a summary of the new plan:
