Dear Members,

Inadvertent Slide Deployment (ISD) PROTOCOL

When an inadvertent slide deployment occurs (ISD), a series of events take place.

First an evaluation is taken to make sure everyone is O.K. The F/A(s) who is/are involved may/will be sent for drug and alcohol testing. Alcohol testing can be up to 24 hrs after the alleged event, the drug testing can be up to 32 hours after the alleged event. It is up to the F/A(s) to contact their US Airways AFA-CWA representative immediately.

Directly after the event, an investigation takes place and a conference call is set up. The individuals on the conference call may include: the Managers of Cabin Safety (or her designee), Ground Operations, Maintenance, Policy and Procedures, a representative from the aircraft manufacturer and a US Airways AFA-CWA ASHS (Aviation Safety, Health and Security) rep. If there is not an US Airways AFA-CWA ASHS rep. on the conference call then the conference cannot take place until one is available.

The objective of the conference call is to gather all the facts about the incident. Nothing from the investigation is ever given to the Inflight dept. Flight attendants are encouraged to be as honest as possible with the details. This part of the process is non punitive. It is common for the F/A(s) and their US Airways AFA-CWA ASHS rep to speak prior to the conference call.

After the investigation is completed, the F/A(s) and their US Airways AFA-CWA rep will set up a meeting with their Inflight supervisor. Flight Attendant(s) could possibly be asked to view a video and view a few questions at that meeting.

Your US Airways AFA-CWA ASHS committee hopes this helps to explain the (ISD) protocol. As always should you have any questions we are more than willing to answer them for you. Slow down your actions and FOCUS. Call your AFA-CWA ASHS rep immediately following any incident.
