In our ever-changing work environment, it can be challenging to keep up with current
Inflight Policies & Procedures (P&P). While flying our trips, committee members are often asked to clarify safety issues. The information in this newsletter came from your questions and suggestions. We encourage open lines of communication. We are not supervisors; we are your flying partners. Like you, we are

It is extremely important that AFA receives copies of these reports. The only way this can happen is if you submit a copy to us. Having a copy of your report allows us to respond to you, track trends, and foresee potential
problems.
You can file a CSR for any safety or security related problem and/or concern. (Some examples include: the agent closes the A/C door without verification that baggage is stowed; you must cut your briefing short due to the pressure to
board.)

When filling out a CSR, DPIR, or SS-194, please print. Report the facts, don't speculate, and don't report your emotional
reactions.
Remember:
DPIR’s may be sent to the FAA and State/Federal Prosecutors.
