accordance with the Voluntary Furlough Letter of Agreement, Paragraph 6, I
hereby request that I be granted an extension of my Voluntary Furlough for the
following duration. I understand that this request is subject to approval by
the Company and that a Voluntary Furlough Extension will not be granted past
December 2, 2005.

form must be received by Inflight Administration no later than 90 days
prior to the expiration date of your Voluntary Furlough.

searchable list on theHub or by calling 1-800-327-0117, Prompts 1, 5, 4, 5. Late requests will not be accepted.
I understand that if awarded a
Voluntary Furlough Extension, I agree to the conditions outlined in the
Voluntary Furlough Agreement between the Association of Flight Attendants and
the Company. I acknowledge that I have
reviewed this agreement, that I be recalled from Voluntary Furlough at any time
if the needs meet all of the eligibility requirements and that I may of the
company dictate.
Furthermore, I understand that
if there are subsequent Involuntary Furloughs, my seniority may no longer allow
me to hold this Voluntary Furlough.
