Subject: Flight Attendant Certification
In the fall of 2003, a flight attendant certification requirement was incorporated under the Vision 100-Century of Aviation Reauthorization Act. In order to comply with this federal certification requirement, you will be required to supply the FAA with specific and detailed information.
US Airways Inflight Services will act as the collection point for that information and forward it on to the FAA for you. Please log on to the Inflight Services home page on theHub and click on the "Flight Attendant Certification" link. Read the information and complete all areas of the form then click the "SUBMIT" button at the bottom.
PLEASE NOTE: The deadline for providing the FAA required information is October 10, 2004.
Once this information has been gathered, it will be sent to the FAA and a certificate will be issued and sent to the permanent address you list on the form.

Flight Attendant Certification Q&A
1. Who is affected by the Flight Attendant Certification Act?
All flight attendants, supervisors, training instructors, and anyone else employed by US Airways who has successfully completed Initial Flight Attendant training.
2. Does this certification process cover only active flight attendants?
If a person completed training on or before the date of enactment (December 11, 2003) he or she is to receive a certificate of proficiency. Current status does not affect this certification process.
3. Who needs to complete the Flight Attendant Certification Form?
ALL current flight attendants (including MidAtlantic flight attendants and all flight attendants on a furlough or leave of absence) MUST fill out the F/A Certification form. In addition, any current US Airways employee who successfully completed Flight Attendant Initial Training may chose to request a certificate.
4. Where can I find this form?
The form will be available only on theHub, Inflight Services home page. All information must be submitted using this form - no hard copies will be accepted.
5. When does this information have to be provided to US Airways?
You must supply ALL information listed on the form to US Airways by October 31, 2004. US Airways will then submit the list to the FAA in time for them to certify each flight attendant by early December 2004.
6. If I have not completed the form/not completed it properly, what will happen?
If US Airways does not receive a properly completed form, you will not receive a certificate. If you are not certificated on or before the date required by this Act, you will not be permitted to exercise the privileges of a flight attendant. Failure to comply could result in disciplinary action up to and including termination of employment.
7. Who is responsible for reporting address changes of to the FAA?
You, the flight attendant are solely responsible for providing updated information to the FAA. If/when your permanent address changes, you must send a signed request identifying yourself by name, social security number, date and place of birth, and certificate number. If your permanent mailing address includes a post office box number, then you must include your current residential address. If your address is listed as a General Delivery, Rural Route (RR), or Star Route (SR), you must provide directions or a diagram for locating the residence attested by your signature. Mail your request to the address below or submit your request on-line at
http://registry.faa.gov. Failure to notify the FAA of a permanent change of address within 30 days could result in the denial of certificate privileges.

Face-to-face with a FAA Inspector, copies of documentation sent to AFS-760, along with a certification statement from the Inspector that the new documents pertain to the flight attendant making application for a new certificate. 9. How do I get a replacement certificate?
If a replacement certificate is needed, send a signed request identifying yourself by name, social security number, date and place of birth, and certificate number. Provide a permanent mailing address, state the reason for needing a replacement certificate and include a check or money order of $2, payable to the Federal Aviation Administration. If your address is listed as a General Delivery, Rural Route (RR), or Star Route (SR), you must provide directions or a diagram for locating the residence attested by your signature. 10. Do I need to carry my certificate with me?
If requested, flight attendants shall present their certificate to the FAA, the National Transportation Safety Board, or another Federal agency within a reasonable period of time after the date of request (FAA policy: 15 days).
11. Can a certificate be revoked for any reason, including lapse of qualification and termination of employment?
A certificate of demonstrated proficiency could be revoked under 14 CFR Part 12 "Investigative and Enforcement Proceedings" (see 14 CFR Part 13.13 Consent Orders).
