2004 Annual Vacation Bid Extension
2004 Vacation Grids have been published in the crew rooms as of
September 30, 2003.
Due to the late date of this publication, we are extending the
Annual Vacation Bid. For those flight attendants who want to add to or
change their bid(s), the vacation screens will remain open until
11:59 PM Sunday, October 5th, 2003. For those of you who do not
want to change you bid, you do not need to do anything.
Please be advised that all bids sent by US Mail and Company Mail
(LOA flight attendants only) must be RECEIVED by Inflight
Administration on or before noon Monday, October 6, 2003.
Although we have delayed the closing of the bid, we will still
award the bid on the published dates and Round Two will open and
close on schedule.
During the month of August, a letter regarding the US Airways
Annual Student Verification was mailed to your home. The letter
outlined the process for you to confirm eligibility for your
dependent student(s) who are between the ages of 19 and 23, or
remove them from coverage due to lack of eligibility. Our
records indicate that you responded to the mailing and verified
that your dependent was no longer a full time student, or that
you did not respond to this mailing. As a result, <Dependent
Name>, was terminated from your health coverage effective August
31, 2003.
You should disregard this notice if you have notified the
Benefit Center within the past two weeks that your dependent
should still be considered a full time student. You can check
the status of your dependent on the web. The URL is:

In accordance with the Consolidated Omnibus Budget
Reconciliation Act (COBRA) of 1985, a letter was sent to your
child regarding their option to continue coverage as an
individual participant through COBRA. Under the provisions of
COBRA, medical and dental coverage may be continued at full cost
in addition to a 2% administrative fee. The letter was addressed
to your child per the requirements of COBRA. If your child is no
longer a full time student and you wish to continue health care
coverage under the provisions of COBRA, you should follow the
instructions included in the letter sent to your child.
Questions about COBRA coverage should be directed to Ceridian,
the COBRA administrator, at 1-877-504-4052.
In the event that your child meets the eligibility provisions
for a dependent student, you are advised to contact the US
Airways Benefit Center no later than October 8, 2003. Questions
regarding this matter should be directed to the US Airways
Benefit Center at 1.800.872.4780, prompt #1.
