Dear Members,

Please be advised that if you are FMLA/PCL approved and you have a sick occurrence that falls under your FMLA/PCL, please ensure that you fax your Doctor's note to Sheila Fraites at 703-872-6476. To ensure that she receives the fax, you can follow-up by contacting her directly at 703-872-6483.

Please keep these numbers for your reference.

The supervisors will elevate you on DCP for your sick that is related to FMLA/PCL without having this documentation on record to Sheila Fraites Office in CCY.

Also as a reminder, The FMLA/PCL must be renewed yearly. Keep track of when you year ends, and if you have the same serious health condition or a different illness that qualifies you for FMLA/PCL, please apply. This is the only way to protect your work record for illness.

Fyi...

There are two different types of PCL (Personal Care Leave) forms. If you are looking for a "block" of time for a medical leave, then ask for the PCL forms and have your Doctor fill out that form that speaks to that "Block of time".

If you are looking for "intermittent PCL leave" your physician fill out the information on the WH380 form that speaks to "intermittent leave" (minimum of 5 days off for medical).

To better protect yourself and your job and if you have a serious health condition that needs covered, I suggest you have your physician fill out both forms.
