Deadline for 2003 Flex Spending Account
US Airways employees who participated in the Health Care, Dependent Day Care Flexible Spending Account Program (FSA) in 2003 should have received a reminder in the mail from Ceridian stating their account balance in the Health Care FSA and/or Dependent Day Care FSA as of December 31. It is vital to submit claims for reimbursement of eligible expenses that were incurred in the calendar year 2003 to Ceridian by March 31. Ceridian will not process reimbursement requests for the 2003 plan year if claims are faxed or postmarked after the March 31 deadline. Employees are strongly encouraged to retain proof of mailing if claims are submitted near the March 31 deadline. Any amounts remaining in 2003 FSA account(s) that are not claimed for reimbursement will be lost.
Here are some important notes for employees on furlough or leave in 2003: Employees who participated in the Health Care FSA Program and elected to continue participation through COBRA can submit such claims incurred throughout 2003. Employees who did not elect to continue coverage through COBRA may only submit for reimbursement claims of expenses incurred through their furlough or leave commencement date. Employees who participated in the Dependent Day Care FSA may only submit claims for reimbursement of expenses incurred through their furlough or leave commencement date, since there was no option to continue this type of FSA through COBRA. Employees who have questions regarding their account should contact Ceridian at 1-877-799-8820. TOGETHER WE FLY TICKETING REMINDER
Employees who want to book travel using the "Together We Fly" program tickets, which were awarded to employees following US Airways' emergence from bankruptcy last March, must do so before June 1. It is important to have your reservations made prior to the June deadline if you intend to use these tickets. Employees are eligible to travel after June 1 provided that advance reservations have been made. As a reminder, reservations must be made 14 days in advance of your trip in "G" inventory.
