VOLUNTARY FURLOUGH EXTENSION NOTICES VF 1-6
VF Extension notifications to flight attendants scheduled to return from Voluntary Furlough in December 2004 have been mailed.
Flight attendants on VF1 scheduled to return in December 2004, have been mailed the following letter. VF 1
participants will not be able to extend and should plan on returning in December. For those flight attendants not interested in returning, it offers the VSIP as an alternative (if eligible).
ATTENTION VF 1 PARTICIPANTS
Dear Voluntary Furlough 1 Participant:
It is hard to believe that we are coming up on 3 years since you were awarded a Voluntary Furlough. As you know, you are scheduled to return from your Voluntary Furlough on December 2, 2004. Under the provisions of the Voluntary Furlough Letter of Agreement, the maximum duration of the program was 36
months, ending on December 1, 2004. Detailed information on the return process will be mailed in mid-September.
We know that a lot has changed here at US Airways and probably in your personal life over the past 3 years, so we'd like to present you with an alternative to returning to work. Enclosed is a copy of the Voluntary Separation Program II 2004. Under the Program, a limited number of Flight Attendants who meet the minimum eligibility requirements may elect to resign from the Company in exchange for:
Certain post-employment travel benefits for themselves and eligible family members. Medical and dental benefits at the employee contribution rate for ninety (90) days.
If you are interested in participating in the program and meet the eligibility requirements, your Application and Release Form must be postmarked no later than September 4, 2004.
Please note that, should the company offer another Voluntary Furlough to offset the return of flight attendants from Voluntary Furlough in December, applicants must be active on the posting day of the bid. In all likelihood, the posting (if needed) would occur prior to your return to active status on December 2, 2004.

Flight attendants on VF2 - VF6 scheduled to return in December 2004 are able to extend for 6 or 12 months. The following letter has been mailed. The extension forms will also be available on theHub in the next couple of days.
ATTENTION VF 2-6 PARTICIPANTS
Dear Voluntary Furlough 2-3-4-5-6 Participant:
I am writing to remind you that you may apply for an extension of your Voluntary Furlough, which is scheduled to expire on December 1, 2004. A copy of the Flight Attendant Request to Extend Voluntary Furlough form is reprinted on the reverse side of this letter. Please note that the deadline for submitting your extension request is Friday, September 3, 2004. You may send your extension form either by mail or by facsimile to Inflight Administration in Crystal City. The address and facsimile number is on the extension form. The results of the award process will be available on Friday, September 24, 2004.
Receipt verification will be available via theHub. Inflight Administration will post a searchable list of forms received on theHub each afternoon (M - F). The list will be available after 5PM and include forms received prior to 11am that day.
Please refer to the Voluntary Furlough Letter of Agreement for details pertaining to your Voluntary Furlough and the extension process, or contact Inflight Administration at 1-800-327-0117, prompt 1,5,4,5, if you have any questions.

Flight attendants on VF7 - VF9 are NOT scheduled to return in December 2004 and therefore are not being sent extension notices.
VOLUNTARY SEPARATION PROGRAM II 2004
The Company is offering the Voluntary Separation Program II 2004, under which qualifying Flight Attendants may leave the service of the Company in exchange for travel benefits. Under the Program, a limited number of Flight Attendants who meet the minimum eligibility requirements may elect to resign from the Company in exchange for certain post-employment travel benefits for themselves and eligible family members. There may be tax consequences associated with the use of these travel benefits.
In order to qualify for the Program, an employee must have completed at least five years of service with the Company as of September 4, 2004. A copy of the Program is available on the Inflight Services page on 'theHub' (thehub.usairways.com).
Please read all of the details of the Program. To be considered for participation, you must complete the Voluntary Separation Program II 2004 Application and Release Form, and return it to:

The application must be postmarked no later than September 4, 2004. Faxes will not be accepted.
If you elect to participate in the Program, you will voluntarily resign your position at US Airways and will not maintain recall rights. You will not be provided with severance benefits, and you will likely be ineligible for unemployment benefits under state law.
