Dear Members,

UHC AND CAREMARK ID CARD UPDATE

The Company distributed information regarding a method to print copies of the United Healthcare ID Card and the Caremark Drug Prescription Card for employees who have not received their cards.

The information was also posted in an AFA Eline and on the AFA website at www.afausairways.org

For those who do not have computer access or have not received their ID cards there is a back up method for providing proof of insurance and prescription coverage.

UHC

Your health care provider can verify proof of insurance by calling the UHC PROVIDER LINE, 877-842-3210. You will need to provide your UHC ID (preferred) number or your social security number and the Group Number for verification.

Your UHC ID number is your original East employee number preceded by 0007. For example my UHC ID number is - 000714113.

The UHC Group number is 704267.

Caremark

The ability to print 2007 Caremark ID cards from the Caremark website will not be available until January 1, 2007. If you have not received a card by that date or experience difficulties printing a duplicate ID card your Pharmacist can call the Caremark Pharmacy Help Desk at 800-345-5413. You will have to provide the pharmacy with the same UHC ID number and the RX Group Number.

The Caremark RX Group Number is T3472007.
