AMENDED PASS POLICY FOR RETIREES AND ELIGIBLE FAMILY MEMBERS

The company will begin implementing some elements of the new combined travel policy on January 1. 2006. As part of this first phase, the boarding priority for retirees will change on that date. A description of the complete combined travel policy can be viewed at:

Please note all travel benefits and boarding priorities contained in our contract such as 25/45 (Section 22J) or negotiated in regard to the Early Retirement Incentive (ERI), Voluntary Furlough with Limited Recall (VFLR), Voluntary Furlough (VF) and Voluntary Separation Programs (VSIP) will remain in full force and effect. 

Here is an excerpt from the December 30 notification regarding retirees. 

Retirees
Beginning January 1, 2006 all retirees travel fees will be waived across the entire route system. Initially, the ID100 paper ticketing process we're using today will apply to all retirees when traveling on the "other" airline. These paper tickets are available at airport ticket counters or the City Ticket Office in Tempe. In a later phase, retirees will travel ticketless.

Beginning January 1, all retirees (except those with a contractual right to fly at active boarding priority) and accompanying family members will travel one boarding priority below active employees. Right now, that means SA3P for West retirees traveling on HP-operated flights and S4 for East retirees traveling on US-operated flights. Travel on the "other" airline will continue to be S5HP for West employees on East flights and SA3C for East employees on West flights until the merged Employee Travel Application (ETA) is activated in a later phase. Unaccompanied family members will travel at one boarding priority below the retiree's status.

The decision on boarding method (DOH or First Come/First Served) for active employees is still pending. Please email your comments to: Row23middle@usairways.com

RESERVE OUT OF BASE

As a reminder.....the Transition Agreement (date January 10, 2005) replaced Section 11.J.1. with the following: "1. A reserve flight attendant shall have a minimum of one hour and thirty minutes (1:30) from the time he/she is awarded/assigned a trip by Crew Scheduling to report to the airport. A reserve flight attendant may reside in a location that is consistent with compliance with this provision given normal surface transportation conditions."

The previous "Reserve Out of Base Policy" will be amended to reflect this change. The Company has informed all Inflight Directors, Managers and Supervisors a reserve flight attendant will not be considered out of base if they reside within 1 and ½ hours of the airport. Please let your local representatives know if you are cited for being out of base for not residing or being in position within one hour of the airport.

EXTENSION OF DEPENDENT VERIFICATION AUDIT DEADLINE

As we notified you earlier the Company has been in the process of requiring employees to verify their dependent eligibility status for health insurance coverage. Budco, the independent auditing firm conducting the audit has extended the deadline for verification documentation until February 6, 2005. Please read the Company message below:

EAST EMPLOYEE DEPENDENT VERIFICATION - DEADLINE IS FEB. 6, 2006
Note: This audit has no impact on employees enrolled in America West health plans. Many employees enrolled in the US Airways East health plans have not yet provided verification of dependents for the Dependent Audit and the original deadline has passed. To give East employees some additional time to submit their documentation, Budco, the independent auditing firm, has granted an extension on the documentation submission deadline, which is now February 6, 2006. This extension does not change the requirement to document/verify dependent status; it simply provides additional time to submit required documentation. Remember that dependents will not be covered retroactively all the way back to January 1, 2005 in cases were there dependent status cannot be verified or is not documented. Got questions? If you do not return the audit along with the appropriate documentation verifying dependent eligibility by the February 6, 2005 deadline you could lose your dependent coverage retroactive to January 1, 2005.
