Dear Members,
Important information about the distribution and content of
healthcare identification cards (2007) and related tips for ordering more cards
or printing temporary ID cards for anyone who wants extra cards for other family
members or for those that do not have access to their home mail box to retrieve
their UHC and Caremark ID cards before January 1. The vast majority of cards
have shipped and delivered already but there may be some later deliveries for
inactive employees whose cards were processed after active employees.

2007 Health Plan ID Cards Started Shipping the Week of December 18, 2006

Most employees already have received them. ‘Received what,’ you ask? Why,
your 2007 UnitedHealthCare and Caremark prescription cards, of course. Your
ticket to good health is in the mail as UnitedHealthCare and Caremark last week
began shipping 2007 medical plan and prescription plan ID cards to the home
addresses of US Airways mainline employees to ensure they’ll be snuggly in your
mailbox – next to all those holiday cards – before the first of the New Year.
Many employees, in fact, already have received them.
Still, we recognize that these are two "never leave home without ‘em" cards
for many of our employees. So if you need cards on January 1 and haven’t yet
seen them in the mail, UnitedHealthCare and Caremark have arranged for you to go
online at www.myuhc.com and
www.caremark.com to print temporary cards
and order more/replacements (sometimes you just need more than two, right?).
Here’s where to go:
For UnitedHealthCare:
Log into
www.myuhc.com. Click on the "Register Now" button or enter your user name
and password. Then click "Manage my Account – Request ID Cards" and follow the
online instructions.
For Caremark:
Log into www.caremark.com. Then,
register for the first time or enter your personal user ID and password, click
on "Manage Your Online Account" and look for "My Coverage." There, you’ll find
"Print ID Card." Click on that and follow the online instructions.
Really, it’s that simple. And in both cases, the new ID cards are valid with
the New Year starting January 1.
About your ID Cards. . .
UHC Medical Plan Cards
These cards carry the UnitedHealthCare logo as well as the US Airways
Benefits US logo on gray, hard "credit card" size plastic that is durable and
convenient. The cards are effective January 1 for all employees who enrolled in
a UHC medical plan or were automatically rolled to this coverage based upon
eligibility and the plan you had in 2006. For West employees these cards are
very similar to 2006 cards. For East employees these cards are new, so here’s a
run down of card features:
- You are the "subscriber" on your new
medical plan ID. Your subscriber number is a nine digit number created by
placing three leading zeroes in front of six digits that are either the
payroll file number if you are a West employee (top left hand corner of your
pay stub) or if you are an East employee, the number seven followed by five
digits that are your pre-merger US Airways employee number.
- The front of the card shows group number,
which is 704267 for all of US Airways. You’ll also see the name of the
covered employee and a list of covered dependents’ names, the company name
and the name of the plan in which you are enrolled. There is a brief summary
of copays that apply for office visit and specialist visits as well as
urgent care and the emergency room.
- On the back of the card you’ll see the
multi-purpose web site address for UHC, the claims address and phone numbers
for providers to call to verify your coverage. Additionally, you’ll find
numbers for UHC Customer Care for you the "consumer", Optum Nurseline,
United Behavioral Health (UBH) for the Employee Assistance Program or
behavioral health services, the international travel network customer
service line (BUPA), Spectera Vision and MetLife Dental customer service.
- If you are covered in a UHC plan you will receive two cards per
shipment. If you need more cards or replacements you can call UHC Customer
Care at 800-520-0811 or go to www.myuhc.com.
Caremark Prescription Cards
These are new for East employees and Caremark is issuing 2007 cards for West
employees as well. All cards are effective as of January 1. These cards are
white, hard plastic ‘credit card’ size and style that is durable and convenient
with the Caremark logo and the US Airways Benefits US logo on the front. The
cards come two at a time affixed to a handy information booklet. They are mailed
to the home address on file for the covered employee. Here are some key features
of the Rx cards from Caremark:
- On the front of card, you’ll find important stuff like your name and ID
number – note that all cards come in your name only – no dependent names are
listed on the card. When a pharmacy plugs in your ID number both you and
your covered dependents, if any, will be verified electronically or on the
phone.
- Your ID number is the same nine digit number described above for your
medical plan identification number with UHC.
- On the back of the card, you’ll find the Caremark customer care phone
number and claims address for the rare occasion when you might need to file
a pharmacy claim directly yourself.
- If you need more Rx cards for larger family sizes or replacements, you
can contact Caremark at 1-866-760-4276 if you are a West origin employee or
1-800-898-5698 if you are an East origin employee or logon to
www.caremark.com to print a temporary
ID card.
The Cards you won’t get. . .
Here’s the list of cards you will not receive.
- There is no separate card for United Behavioral Health services or
Spectera Vision program. The information for these services is on the
reverse of your UHC medical plan ID. If you want additional information on
UBH services, call the UHC customer care line at 800-520-0811 or log onto
www.liveandworkwell.com using
the access code ‘US Airways.’ There is also a separate phone number
exclusively for EAP/Behavioral Health services through UBH – call
800-363-7190.
- MetLife Dental does not require ID cards. For information on service
providers and claim status updates, call 800-942-0854 or log onto
www.metlife.com/dental.
