Dear Members,
New Jumpseat Forms
Effective December 6th, 2006 a revised jumpseat form was introduced system wide.
This same form is being used by the pilots as well.
The procedures remain the same for requesting a cabin jumpseat at the gate on
East or West operated flights.
-- Complete the front side of the form, with the exception of: "Cass Approved",
"Agent USE Only", and "Boarding Category" line boxes.
-- The "Position" line (front of form) refers to your job title at US Airways
i.e., Flight Attendant.
-- The "Boarding Priorities" list on the back of the form are used for flight
deck jumpseats only.
-- Introduce yourself to the flight crew when boarding, show your company ID and
give the white copy of the form to the flight deck. Keep the pink copy of the
form.
An In-Flight Systems HotSheet will be available in all bases for Flight
Attendant reference.
