HEALTH INSURANCE FOR FLIGHT ATTENDANT'S RETURNING FROM A VF

A special reminder for any flight attendant who is returning from a voluntary furlough, you MUST contact the benefits center by December 30, 2005 if you want your health insurance to continue. IRS regulations require that employees who are returning from a leave that crosses a calendar year must re-sign up for their benefits. If you want to continue health insurance coverage in 2005 and have health insurance coverage in 2006, you MUST call the benefits center no later than December 30, 2005. Failure to do so will result in the cancellation of your health insurance with US Airways and you will not be able to obtain employer group health insurance coverage from US Airway until January 2007.
